Indiana PathWays for Aging Provider
Caring for family is real work. Indiana may pay you for it.
If you're caring for your mom, your dad, your spouse, or another loved one at home, Structured Family Caregiving through Indiana PathWays for Aging may allow you — the family caregiver — to be trained, supported, and paid for the care you're already giving.
Hearts Open Home Care is a family-owned Indiana home care agency — owned by Yolaikatia and Troy Hatchett right here in Hendricks County. We're not an app and we're not a call center in another state. We're a family that cares for families, and we walk this whole process with you, in person, from the first phone call.
Not sure if you qualify? That's exactly what the first conversation is for. The program makes the eligibility decision — our job is to help you find out, honestly, and to handle the process with you if you do.
Who this is for
Structured Family Caregiving is worth exploring if:
- You live with (or are preparing to live with) a parent, spouse, or loved one who needs daily hands-on help — bathing, dressing, meals, moving safely around the house.
- You're already doing that care unpaid, and it's costing you a job, hours at work, or your own health.
- Your loved one is on Indiana Medicaid, or may qualify for Indiana PathWays for Aging, the state's program for Hoosiers 60 and older who need nursing-home-level care but want to stay home.
- You're an adult child in another state trying to set up real support for a parent back home in Indiana — see our note for far-away families below.
You do not need a nursing background. Family caregivers come from every walk of life. What you need is a loved one who qualifies through the program and a willingness to be trained and supported — and that's where we come in.
Straight talk:
We can't promise you'll qualify, and nobody honest can. Eligibility and payment are decided by the program, based on your loved one's needs and Medicaid status. What we can promise: a real conversation, a clear explanation of your options, and a local team that does the paperwork with you instead of handing you a portal login and wishing you luck.
How it works
Step 1 — Talk with us (free).
Call (317) 992-1944 or book a free phone or virtual consultation. You'll talk with our Onboarding Specialist — a real person, here in Hendricks County. We answer the same day you reach out, and if it makes sense to move forward, we offer an assessment within 48 hours.
Step 2 — Find out if your loved one qualifies.
Structured Family Caregiving runs through Indiana PathWays for Aging. The state and its care-management partners assess your loved one's level of care and Medicaid eligibility. We help you understand what's being asked, gather what's needed, and keep the process moving — and we tell you plainly if it's not the right fit or the timing isn't right.
Step 3 — Get set up as a Hearts Open family caregiver.
If your loved one is approved and you choose Hearts Open, our Hiring Team walks you through onboarding: background checks, required training, and the day-to-day expectations of the program. You're never handed a binder and left alone with it.
Step 4 — Care with a local team behind you.
You care for your loved one at home, as part of the Hearts Open team: ongoing coaching and check-ins from your Onboarding Specialist, care planning support, help documenting the care the program requires, and people who answer the phone when something changes at 9 p.m. Payment for your caregiving is set by the program's rules — we'll explain exactly how it works in your situation before anything is signed.
What Hearts Open does for family caregivers
Named local people.
Your onboarding specialist is Yushawn. Your hiring and HR contacts are Troy Hatchett and Kat Hatchett. Your owners are Yolaikatia and Troy — a Hendricks County family. You'll know every name before you need it.
The paperwork, walked with you.
Applications, documentation, care logs, renewals — we sit on your side of the table and work through it with you, in plain language.
Training that respects you.
You already know your loved one. We add the skills the program requires — safe transfers, daily care routines, what to document and why.
Same-day answers.
Questions don't wait for business hours in a caregiving house, so we don't make you wait either. Call, and a person answers.
Honesty about fit.
If Structured Family Caregiving isn't available to your family, we'll tell you what is — including our regular home care services — and never pressure you into the wrong door.
For the kids far away
You're the one Googling at midnight. Your parent is here in Indiana; you're not — and "I hope they're okay" is not a plan.
If your parent may qualify for PathWays, Structured Family Caregiving could let a family member close to them be supported and paid — and we'll keep you in the loop with regular updates, so distance stops meaning helpless. Book a virtual consultation and talk it through with Yushawn from wherever you are.
